Required
A valid resale certificate or state sales tax exemption number is required to open a wholesale account.
Wholesale Application
All fields are required unless marked optional.
Missouri accounts: Missouri Form 149. Out-of-state accounts: provide your home state equivalent.
By submitting this form you confirm that you hold a valid resale certificate or sales tax exemption for your state. After submitting, please email a copy of your resale certificate to wholesale@saintheck.com to complete your application. Saint Heck LLC reserves the right to approve or decline wholesale accounts at our discretion. We'll follow up within 1-2 business days.
